The body-weight evidence for psyllium is weak and contradictory. Examine grades the primary clinical outcome Weight as D (No effect, 2 studies / 465 participants), and the PubMed evidence corroborates this: a 2019 dose-response meta-analysis of 22 RCTs found no significant effect on body weight (MD -0.28 kg, NS), BMI or waist circumference, and the largest and most recent 2025 meta-analysis of 27 RCTs actually found a significant INCREASE in body weight (WMD +3.57 kg), partly attributed to gel water retention. The single favorable meta-analysis (2023, MD -2.1 kg) was authored by Procter and Gamble employees, the manufacturer of Metamucil, a clear industry conflict of interest, and is rated low quality. No professional body (The Obesity Society, AACE) endorses psyllium as a weight-loss treatment, and Mayo and Harvard do not address it as a weight-loss intervention at all. The only consistent and significant signal is a short-term satiety/appetite effect, which is a surrogate outcome that does not translate to durable body-weight reduction. A grade of D (no demonstrated effect on the primary clinical outcome) is the honest reading.
