The 2023 joint AGA-ACG Clinical Practice Guideline explicitly names psyllium and identifies it as the only fiber supplement found effective for chronic idiopathic constipation, and a 2022 updated meta-analysis (PMID 35816465, 16 RCTs) ranked psyllium as the most effective fiber type, with the psyllium subgroup showing a significant increase of about 3 bowel movements per week and improved stool consistency. However, the grade is held at B rather than A because the AGA-ACG recommendation is conditional and based on explicitly low certainty of evidence, the psyllium subgroup rests on only 3 RCTs, a 2025 elderly meta-analysis found no significant improvement in stool frequency, and the effect on stool consistency and other constipation symptoms is more modest than on frequency. Examine grades the relevant constipation outcomes only C due to limited RCT count, which is consistent with a B (well-supported but not definitive) overall judgement rather than an A.

Higher gastrointestinal side effects (bloating, flatulence) noted.

Psyllium is superior to docusate sodium for treatment of chronic constipation

結論:Multi-site, randomized, double-blind, parallel-design trial in chronic idiopathic constipation. Psyllium increased stool water content significantly more than docusate (2.33% vs 0.01%, p=0.007), and also increased stool water weight, total stool output and objective constipation measures. Psyllium was superior to docusate sodium for softening stool and had greater overall laxative efficacy.
