The evidence base for cereal (oat/barley) beta-glucan lowering LDL cholesterol is exceptionally consistent: four high-quality meta-analyses spanning 58+ RCTs and over 6,000 participants converge on an LDL reduction of roughly 0.19-0.27 mmol/L (about 5-10%) at a daily dose near 3 g, with similarly consistent total-cholesterol reductions. This consistency is reinforced by an unusually strong layer of regulatory and professional backing: an FDA Significant Scientific Agreement authorized health claim (21 CFR 101.81), EFSA Article 13(1) and Article 14 authorized claims, and explicit supportive positions from the AHA and NLA. However, every graded outcome is a surrogate lipid biomarker, not a hard cardiovascular endpoint (MI, stroke, cardiovascular mortality), and the absolute effect is modest and adjunct-sized rather than therapeutic. Because the totality is best characterized as strong, consistent, regulator-endorsed surrogate-marker evidence with a genuinely modest effect and no hard-endpoint confirmation, an independent grade of B is most defensible; an A would over-state certainty by treating surrogate-only evidence as if it were outcome evidence. The grade applies specifically to CEREAL (oat/barley) beta-glucan and must not be transferred to yeast/fungal beta-glucan, whose cholesterol evidence is not graded here.
