The body of evidence for MCT and weight loss consists of small, short (mostly 4-16 week), heterogeneous, low-to-moderate quality RCTs. The pivotal 2015 meta-analysis of 13 RCTs (n=749) found only a ~0.5 kg body-weight reduction, and the authors themselves explicitly cautioned that the evidence is not of the highest quality. Critically, this is a SUBSTITUTION effect (MCT replacing long-chain triglycerides), not a benefit from adding MCT oil on top of a normal diet — and MCT oil is calorically dense (~8.3 kcal/g), so additive use can promote weight gain. EFSA rejected the body-weight health claim in 2011 for failure to establish a cause-and-effect relationship, and major obesity societies (The Obesity Society, AACE) are entirely silent on MCT, declining to elevate it to a recommended therapy. A real but small and clinically marginal effect under specific substitution conditions, with weak evidence quality and no regulatory or society endorsement, warrants Grade C rather than the B assigned by Examine.
