理由如下:(1) 證據量大但品質參差。

L1 Examine 給 Grade A(54 RCTs / n=6732,Moderate Improvement),但這是依研究數量而非 GRADE 確定性所評;L2 PubMed 5 篇 2022-2025 MA 雖一致顯示 RR 1.40(95% CI 1.18-1.66)對全球症狀有顯著正向效果,且腹痛 SMD -0.94,但 Cochrane 式 GRADE 確定性多為 low to very low(37541528 明示 moderate-low-very low),主因菌株異質性極大、廠商資金未充分揭露。

(2) L4/L5 監管與學會立場明顯不一致:L4d TFDA 健字號核可「胃腸功能改善」(supportive),但 L4b EFSA 拒絕所有相關健康宣稱(against),L4a FDA 對嬰幼兒發出警告但對成人 IBS 無立場;L5a NIH ODS 與 L5d Harvard(Anthony Lembo MD)皆為 may consider 之保留語氣;L5e AGA 2020 GRADE 指引明確 against(『僅在臨床試驗情境下使用』),L5b Mayo 與 L5c Cleveland 也都 cautious。

(3) L1 Grade A 與 L5e AGA AGAINST 之間落差超過 2 階,符合 SOP 中 dispute 觸發條件,但因機轉合理、症狀型疾患安全性高、且 L2 多 MA 方向一致,故不全盤否決而採 B(may consider)。

(4) 在台特殊情境:TFDA 健字號為產品認證而非療效背書,市場過度宣稱『改善便祕腹瀉、打造易瘦腸』已多次裁罰;菌株標示不實案例(LAC 標 250 億實測 69 億)也降低市售品可信度。
