The body of human evidence consistently fails to support MCT oil as an endurance ergogenic aid. The most authoritative source, a 2022 systematic review of 13 human studies (PMID 36096496), found very-little-to-no effect on endurance performance, respiratory exchange ratio, glucose, or fat/carbohydrate oxidation, and concluded MCT-derived ketones cannot serve as a primary exercise fuel. The classic glycogen-sparing mechanism is largely unsupported, and the only positive finding is a single unreplicated 6-subject cycling crossover (PMID 8806933) offset by larger null trials. Cleveland Clinic explicitly cites the same 2022 review and is skeptical; ODS/NCCIH call the evidence insufficient; ACSM and ISSN issue no endorsement; Mayo Clinic and Harvard have no coverage. GI distress is common above ~30 g and can itself impair performance. A D grade (evidence shows no meaningful benefit / claim essentially debunked) is the honest grade.
