The evidence for lutein and cognitive function is genuinely preliminary. Four small short-to-medium-term RCTs (n approximately 44 to 90, mostly 52 weeks) report domain-specific improvements such as complex attention, cognitive flexibility, visual episodic memory and visual learning, supported by fMRI signals of increased prefrontal activation and perfusion, but a 2021 meta-analysis of RCTs found no statistically significant overall cognitive effect and one 52-week brain-morphology trial showed negligible between-group differences. Several trials carry industry conflicts of interest (corresponding author tied to a sponsored contract research organization; an author employed by Abbott Nutrition), lutein was almost always co-administered with zeaxanthin so the effect cannot be isolated, and no medical society endorses lutein for cognition. Examine has no coverage for this pairing (coverage gap, not a verdict), and both the Alzheimer's Association and American Academy of Neurology hold that no supplement is proven to benefit cognitive function. Evidence exists and is non-trivial but is small, inconsistent and uncertain, which fits grade C rather than B or U.
