Psyllium fiber for cholesterol is unusually well-substantiated for a dietary supplement: four independent meta-analyses (up to 41 RCTs, ~1900-2000 participants) consistently show a significant LDL-C reduction of roughly 8-13 mg/dL (~7%), with concordant non-HDL-C and apoB reductions. The mechanism (bile-acid binding) is well understood, and the finding is corroborated by formal regulatory backing rare among supplements: the US FDA authorized health claim under Significant Scientific Agreement (21 CFR 101.81) and an EFSA-substantiated EU-authorized claim, plus explicit naming by the AHA and NLA. However, all outcomes are surrogate lipid markers, not hard cardiovascular endpoints (MI/death), the effect size is modest and adjunct-sized (not a statin replacement), triglyceride and HDL-C effects are null, and one statin-adjunct meta-analysis was small and industry-funded. A grade is therefore strong but not A: the totality supports a solid B, matching Examine's own B grade for the primary LDL outcome.
