Lutein/zeaxanthin significantly slowed progression to advanced AMD in the pre-specified lowest-dietary-intake subgroup (HR 0.74) and was significantly superior to beta-carotene in a direct comparison (HR 0.82 for late AMD, HR 0.78 for neovascular AMD), with the 10-year follow-up confirming benefit and the absence of lung-cancer risk. Authoritative bodies are consistent and supportive: AAO's Preferred Practice Pattern formally recommends the AREDS2 formula, NEI quantifies a ~25% risk reduction in low-intake individuals, and NIH ODS, Mayo, Cleveland Clinic and Harvard all endorse it for intermediate/advanced AMD. The grade is held at B rather than A because the AREDS2 primary intention-to-treat analysis did not reach significance overall (HR 0.90, P=.12), the significant benefit comes from subgroup and active-comparator analyses, lutein was delivered within a multi-component formula, and the 2017 Cochrane review rated standalone lutein/zeaxanthin evidence as low-certainty with limited effect.
