The totality of evidence for resveratrol in type 2 diabetes is genuinely conflicted and does not support a confident positive grade. The two highest-quality syntheses point AGAINST a meaningful benefit: the Cochrane review (PMID 31978258) found null effects on HbA1c, fasting glucose and HOMA-IR, and the high-quality umbrella review (PMID 34320173) explicitly concludes current evidence does NOT support resveratrol for cardiometabolic management in T2D; the largest single RCT (PMID 27520400, n=192, 6 months) was also null at both 40 mg and 500 mg/day. Conversely, several moderate-quality meta-analyses report statistically significant reductions in HbA1c, HOMA-IR and fasting glucose, but these effects are small, heterogeneous, and concentrated in high-dose (>=1000 mg/day) subgroups and short trials (<12 weeks) that few real-world products or users achieve. Examine grades the glycemic outcomes B (Small Improvement), but this conflicts with the Cochrane/umbrella conclusions and the ADA position, so I weight it down. Given the unresolved conflict, the dependence of any benefit on doses well above the EU/Taiwan regulatory cap of 150 mg/day, and the absence of any society endorsement, the honest grade is C: evidence exists but is small, inconsistent and uncertain.
