The evidence for alpha-lipoic acid in type 2 diabetes is confined entirely to surrogate glycemic markers (HbA1c, fasting plasma glucose, HOMA-IR), with no primary clinical-outcome data on diabetes endpoints. The meta-analytic picture is inconsistent: the largest dose-response meta-analysis (PMID 36006850, n=1035) found statistically significant but sub-MCID reductions (HbA1c approx -0.17% per 500 mg/day, FPG approx -6 mg/dL) that the authors themselves judged not clinically important, while a meta-analysis of uncomplicated T2D (PMID 32184879, n=553) found no FPG or HbA1c benefit versus placebo at all. NCCIH/NIH concludes ALA is not superior to placebo for blood-glucose control, and neither the ADA nor EASD endorses any supplement for glycemic benefit. Because the effect is surrogate-only, modest, frequently below clinical relevance, and inconsistent across MAs, a B grade overstates the evidence; C (limited, mixed, surrogate-only) is the honest grade. ALA is not a primary diabetes therapy and carries a real additive-hypoglycemia interaction with antidiabetic drugs plus the insulin-autoimmune-syndrome safety signal.
