The PubMed search (L2) found no dedicated serrapeptase-for-OA randomized controlled trial; the frequently cited Klein and Kullich (2000) knee-OA enzyme trial actually used Phlogenzym (a bromelain/trypsin/rutin combination), not serrapeptase, and is a miscitation that must not be counted. Examine has no osteoarthritis or joint-pain entry for serrapeptase at all (L1). The 2013 systematic review (PMID 23380245) graded the entire serratiopeptidase evidence base as methodologically poor and insufficient to support its use as an analgesic, and the agent's prescription-drug approval was withdrawn in Japan in 2011 for failing to beat placebo, with Taiwan's regulator likewise concluding insufficient efficacy and declining license renewal. No medical institution (Mayo, Cleveland, Harvard) or society (ACR, OARSI) addresses serrapeptase for osteoarthritis. With zero condition-specific trials and a parent evidence base judged poor by regulators and reviewers, the only honest grade is Unproven / Insufficient (U).
