Diabetic peripheral neuropathy is alpha-lipoic acid's strongest indication and a genuine primary clinical endpoint, but the evidence is sharply route-dependent. Four meta-analyses (>4000 participants) consistently show IV ALA 600 mg/day for 3 weeks produces a clinically meaningful symptom reduction (TSS SMD up to -3.59), yet the oral route that consumers actually take produces only a small, often sub-clinical effect (oral TSS SMD -0.46), no significant benefit on objective nerve-conduction measures, and a 2024 Cochrane review concluded oral ALA probably has little or no effect on symptoms at 6 months. Foundational ALADIN/SYDNEY/NATHAN trials were ALA-manufacturer-funded, the AAN rates the evidence insufficient (Level U), and the ADA omits ALA entirely from recommended neuropathy therapies. Examine's B grade is defensible for symptom relief as a whole, so I retain B for the oral-supplement claim, but it sits at the low end of B — capped by the IV-vs-oral efficacy gap, industry COI, and the absence of any specialty-society endorsement.
