The totality of evidence is insufficient to grade TUDCA for NAFLD/MASLD on any meaningful clinical endpoint. There is no completed human trial testing TUDCA against NAFLD histology, liver fat imaging, or fibrosis. The single relevant human RCT (Kars 2010, n=20, 4 weeks) measured hepatic and muscle insulin sensitivity, which is a surrogate for insulin resistance rather than a NAFLD outcome, and it found no change in ER stress markers, undercutting the proposed chemical-chaperone mechanism in humans. All NAFLD-specific efficacy data (steatosis, fibrosis, gut-liver axis) come from mouse models. No regulator (FDA, EFSA, NHS, TFDA, WHO) and no clinical institution or professional society (Mayo, Cleveland Clinic, Harvard, AASLD, AGA) endorses TUDCA for NAFLD, and the closely related parent compound UDCA explicitly failed to show histological benefit in NASH RCTs. Examine's C grade rests on a fuzzy-mapped Obesity 'Liver Cell Content' row (1 study, n=5) that is not a NAFLD endpoint, so a C grade overstates the directly applicable evidence; U is the honest grade.
