關鍵理由:(1) **FAZST 2020 (PMID 31910279, JAMA, n=2370) 為男性不孕症補充劑領域少數雙盲、安慰劑對照、政府資助、多中心、足量、夠長(6 個月、涵蓋一個完整精子生成週期)的金標準 RCT**,其 primary endpoint(9 個月內累積活產率)完全陰性(34% vs 35%, RD -0.9%, 95% CI -4.7 to 2.8),且**所有**精液參數(濃度、總動力、形態、體積、總活動精子數)皆 n.s.;DNA 碎裂率反而偏向 placebo(+2.4%, 95% CI 0.5-4.4)、GI 副作用顯著增加。

此單一試驗的方法學品質與樣本量足以推翻先前 FAZST 之前所有小型 open-label 陽性試驗的訊號。

(2) 後續證據持續支持 FAZST 結論:2023 Heliyon MA (PMID 37539255, n=2168) zinc+folic acid 子組對動力(MD +1.67, 95% CI -1.29 to 4.63, p=0.27)、濃度、形態皆 n.s.;2025 Aalborg MA (PMID 40431450, 50 RCTs) 結論「沒有任何膳食補充品對活產或懷孕有令人信服的效果」、GRADE generally low/very low。

(3) Cochrane 2022 整體抗氧化劑陽性訊號不能歸因於 zinc — zinc 未單獨報告 subgroup,移除高偏差研究後活產 OR 1.22 (95% CI 0.85-1.75) 不顯著、GRADE very low。

(4) L1 Examine 在 Infertility 條目下僅給 Seminal Motility C 與 Sperm Count C(皆 single study, n=2168 — 即 FAZST/Heliyon 數據池),且為 surrogate;最重要的活產/懷孕 outcome 完全未列。

(5) 學會與聯邦層級立場一致:ASRM/AUA Guideline Statement 45「questionable clinical utility」明確不建議例行使用、NIH ODS「inconsistent and inconclusive ... does not recommend routine zinc supplementation for male infertility outside of documented deficiency」。

Mayo cautious_supportive 與 Cleveland conditional_neutral 之亞組支持只限 zinc-deficient / MOSI 族群,不涵蓋一般想生育男性。

結論:Zinc + folic acid did NOT improve live birth (404/1185, 34% vs 416/1185, 35%; risk difference -0.9%, 95% CI -4.7 to 2.8). NO significant differences in any semen quality parameter at 6 months: sperm concentration, total motility, morphology, semen volume, total motile sperm count all n.s. between groups. DNA fragmentation was paradoxically HIGHER in the supplementation arm (29.7% vs 27.2%, MD +2.4%, 95% CI 0.5 to 4.4). GI side effects more common with active treatment (abdominal pain 6% vs 3%; nausea 4% vs 2%; vomiting 3% vs 1%). Authors conclude findings do not support use of folic acid + zinc supplementation in male infertility.

結論:Antioxidants overall may increase live birth (12 RCTs, n=1283: OR 1.43, 95% CI 1.07-1.91) and clinical pregnancy (20 RCTs, n=1706: OR 1.89, 95% CI 1.45-2.47, p<0.00001). When high-risk-of-bias studies removed, live birth signal lost significance (OR 1.22, 95% CI 0.85-1.75, p=0.27). GRADE: very low-certainty for live birth, low-certainty for pregnancy. Zinc-specific subgroup analyses for live birth/pregnancy not reported. Mild GI side effects more common with antioxidants overall (OR 2.70, 95% CI 1.46-4.99).
