The largest infertility study (PMID 20348942, n=350) is open-label and uncontrolled with only 75/350 completing analysis, so its reported semen-parameter gains and 14.7% spontaneous pregnancy rate cannot be attributed to the extract. The single true RCT (PMID 23243445, n=109) was powered for quality-of-life endpoints and reported sperm-motility (+44.4%) and semen-volume (+18.2%) gains only as secondary within-group changes, with no pregnancy or live-birth outcomes and unclear between-group significance. No meta-analysis pools fertility endpoints; the 2022 meta-analysis covers testosterone only. Examine grades the fuzzy-matched Sperm Quality and Seminal Motility outcomes C (1 study, 109 participants each), and AUA/ASRM treat fertility supplements broadly as of questionable clinical utility with inadequate evidence to recommend specific agents, while EFSA's genotoxicity refusal and documented heavy-metal contamination add real safety concerns.
