(2) L2 PubMed 證據對 surrogate(精液參數)一致正向:4 篇 meta-analysis(PMID 40126496 2025、40350672 2025、33906513 2021、33405232 2021)重複驗證 NAC 600 mg/day 對 sperm concentration(MD +3.80~+4.43 ×10⁶/mL)、total motility(MD +4.69~+9.69%)、normal morphology(MD +1.36~+1.68%)、ejaculate volume(MD +0.69 mL)皆顯著改善(p 多 <0.001);DNA fragmentation 亦有 RCT 證據(PMID 30771790 與 33098382 將 DFI 由 19.34% 降至 15.14%, P<0.001)。

(3) **但 patient-relevant endpoint(活產/受孕率)證據薄弱**:所有 NAC-specific MA 均未 pool live birth 或 clinical pregnancy 為主要結果;2022 Cochrane antioxidants 整體活產 GRADE 評為 very low-certainty、移除高偏差研究後 OR 1.22 不顯著、且 NAC 未單獨 subgroup pool。

(4) NIH/NCCIH 直接陳述「research is too limited for any conclusions」(stance_inferred=insufficient);Mayo 因主要 URL 403 fallback、無 Mayo 權威文本、score 受限 ≤0.7;Cleveland not_addressed;Harvard no_position;ASRM/AUA Guideline Statement 45『questionable clinical utility』與 CoQ10/Zinc 同(不點名 NAC,僅在抗氧化劑類別層級不推薦)。

(5) 有利於 NAC 的特殊訊號:2025 PMID 40350672 直接驗證 NAC monotherapy 已具效果,無須 cocktail;2025 PMID 40126496 在 5 篇 RCT 池中未報重大 AE,安全性訊號良好。

(6) 與 CoQ10 對比:CoQ10 surrogate 為 Examine B、有獨立 grading 支持;NAC 在 Examine 完全空白,故 surrogate 證據雖有 4 篇 MA 支持但缺 supplement aggregator 背書,反而稍弱。

(7) 與 Zinc 對比:Zinc 因 FAZST 2020 (n=2370) 已有決定性陰性試驗,故獨立分級可給 D;NAC 沒有對應規模的雙盲、安慰劑對照、活產為主要 endpoint 的 confirmatory trial — 因此 NAC 仍屬『surrogate 強、patient endpoint 不確定』的中間地帶,不能等同 Zinc 的『證據明確指向無效』。

獨立分級給 C(薄弱證據,僅 surrogate 一致正向、患者終點未驗證、學會層級不推薦),低於 Examine 整體的 supportive_with_nuance(因 Examine 此 condition 條目空白)、與 L2 PubMed 的 supportive 平衡、且高於 Zinc 的 D(因 NAC 沒有 FAZST 等級的決定性陰性試驗)。

Meta-analysis of the efficacy and safety of L-carnitine and N-acetylcysteine monotherapy for male idiopathic infertility

結論:NAC monotherapy significantly improved sperm concentration, total motility, normal morphology, and ejaculate volume (volume p=0.002) versus placebo. Both NAC and L-carnitine monotherapy were effective; L-carnitine additionally raised testosterone (p<0.001). NAC did not significantly alter testosterone, LH, FSH, or prolactin. Authors conclude both single agents are effective without need for a cocktail to obtain semen-parameter benefit. Live birth/pregnancy not primary endpoint.
