The evidence for lactoferrin in immune function is population-dependent and dominated by a contradiction between small positive trials and the single best-powered hard-endpoint trial. The largest and best-designed RCT, ELFIN (PMID 30635141, n=2203, UK NIHR-funded), was NULL for late-onset sepsis in very preterm infants (adjusted RR 0.95, 95% CI 0.86-1.04, p=0.233), directly contradicting the earlier smaller Manzoni LISA trial (n=472, RR 0.34) which had reported a large benefit. Meta-analyses are split by age: the 2022 systematic review (PMID 35481594, 25 studies) showed RTI reduction in infants and children (OR 0.78) but no effect in adults (OR 1.00); a single small preschool RCT (PMID 38397361, n=50) reported a 50% episode reduction but is too small and single-centre to be confirmatory. No major authority endorses lactoferrin for immune function: WHO does not address it; CDC, NHS, Mayo Clinic, Cleveland Clinic and Harvard Health publish no recommendation; the American Academy of Pediatrics has not endorsed routine preterm-infant supplementation post-ELFIN; NIH ODS has no dedicated fact sheet; EFSA has authorised bovine lactoferrin only as a safe novel food, with no Article 13/14 health claim. Regulatory acceptance is purely as a food ingredient (FDA GRAS, EU novel food, TFDA additive 「08112」), not as an evidence-based immune therapy. A B grade would overstate a body of evidence whose largest hard-endpoint trial is null and whose adult RTI signal is null; C (limited, mixed, population-dependent, no society endorsement) is the honest grade. Lactoferrin is also absent from the Examine database entirely, so there is no aggregator-level grade to reconcile with.

結論:ELFIN was NULL: 316/1093 (29%) in lactoferrin group vs 334/1089 (31%) in placebo developed late-onset sepsis; adjusted RR 0.95 (95% CI 0.86 to 1.04, p=0.233). No reduction in morbidity or mortality. Authors concluded the data do not support routine use to prevent late-onset infection in very preterm infants.

Lactoferrin in the Prevention of Recurrent Respiratory Infections in Preschool Children: A Prospective Randomized Study

結論:Lactoferrin group had 50% reduction in median respiratory infection episodes (1 vs 2, p=0.02); 80% lower odds of upper RTI; corticosteroid use 32% vs 60% (p=0.047). Symptom duration and school absence also reduced.
