The evidence base for nattokinase lowering blood pressure is small, internally inconsistent, and not endorsed by any major body. A 2023 meta-analysis pooling only 6 RCTs (546 participants) reports a modest, statistically significant reduction (SBP about -3.45 mmHg, DBP about -2.32 mmHg), and two small short trials (2008 South Korea n=86, 2016 North America n=79) show a directionally similar effect. However, the largest and longest trial (NAPS 2021, n=265, median 3 years) found no significant blood-pressure effect, directly contradicting the shorter trials, and individual studies are mostly n<90, heavily East-Asian, with funding largely undisclosed and manufacturer conflict of interest unexcludable. Examine grades Blood Pressure C on a single n=86 study; no health authority (FDA, EFSA, NHS, WHO, TFDA) authorises a blood-pressure claim, and neither the AHA nor any hypertension society addresses or endorses it. A modest signal with a contradicting pivotal trial and no authoritative support is a textbook C: limited, suggestive evidence that is not strong enough for B and not absent or refuted enough for D.

Nattokinase atherothrombotic prevention study: A randomized controlled trial
PMID: 33843667 2021 RCT (double-blind) n = 265
結論:Over a median 3 years in healthy individuals at low CVD risk, there was no significant effect of nattokinase on blood pressure, and a null effect on subclinical atherosclerosis progression. This is the largest and longest trial and contradicts the BP signal seen in shorter trials of (pre)hypertensive patients.
