The evidence for astaxanthin in eye fatigue / digital eye strain consists of roughly five small Japanese RCTs (n approximately 40 to 64, 4 to 12 weeks) that directionally agree on improving subjective fatigue questionnaire scores, which keeps this above a D, but every quality marker is weak. Every trial was funded by an astaxanthin raw-material or supplement company (Fuji/AstaReal, BGG Japan, DHC, FANCL) and several list manufacturer employees as authors, creating a severe and uncorrected conflict-of-interest and publication-bias risk. Several positive trials used multi-ingredient formulas (lutein, anthocyanin, DHA) so the effect cannot be attributed to astaxanthin alone, and objective visual indices (acuity, refraction, accommodation parameters) often failed to reach significance while only subjective questionnaires improved. Examine does not list any eye-fatigue/strain/accommodation outcome for astaxanthin at all (found_in_examine=false), EFSA explicitly rejected the astaxanthin visual-acuity health claim for insufficient evidence, and no ophthalmology or optometry society (AAO, AOA) endorses any supplement for digital eye strain, with the AAO journal's 2022 systematic review finding no high-certainty evidence for any CVS therapy. The totality is consistent-direction but small, industry-bound, low-quality, and unendorsed evidence: grade C.
