Four independent meta-analyses spanning 7 to 35 RCTs (the 2023 GRADE-assessed review pooled 1,076 participants) all converge on the same direction: spirulina significantly lowers total cholesterol, LDL-C and triglycerides, with a smaller and less consistent HDL-C rise. This directional consistency across multiple pooled analyses is the strongest argument and supports a moderate grade. However, the evidence falls short of grade A: effect-size estimates vary enormously across MAs (TC reduction from roughly -11.5 to -46.76 mg/dL), the underlying trials are small, short, drawn from a narrow set of research groups with largely undisclosed funding, and heterogeneity is high. The most recent and largest MA (35 trials) shows markedly more modest effects, suggesting early small-trial MAs overstated magnitude. Guideline non-endorsement (AHA states dietary supplements are not recommended for cholesterol management; NLA silent) and the absence of independent large confirmatory trials cap the grade at B rather than A, while the consistent significant signal keeps it above C.
