All cardiovascular evidence for pomegranate rests on surrogate markers, with no trial reporting hard outcomes (myocardial infarction, stroke, cardiovascular mortality). The strongest signal is blood-pressure reduction: two meta-analyses (8 and 22 RCTs) consistently show SBP falling roughly 5-8 mmHg, but both carry very high statistical heterogeneity (I-squared near 90%) and largely undisclosed funding, so industry conflict of interest cannot be excluded. Crucially, the only adequately powered randomised controlled trial of carotid intima-media thickness (PMID 19766760, n=289, 18 months) was NEGATIVE on its primary endpoint, with benefit appearing only in post-hoc exploratory subgroups, and the widely cited 30%-CIMT-reduction study (PMID 15158307) is n=10, non-randomised and unblinded - low quality with high bias risk. A 12-RCT lipid meta-analysis found no significant effect on cholesterol or triglycerides, and EFSA rejected every pomegranate cardiovascular-related health claim while FDA issued a warning letter against exactly these atherosclerosis and blood-pressure claims. The totality is genuine but small, inconsistent and surrogate-only evidence with a failed pivotal hard-ish endpoint - a clear C, not B.
