The evidence for black seed (Nigella sativa) as an asthma intervention is real but limited. Two meta-analyses (PMID 34658694, 31892440) pooling only 4 RCTs and roughly 187 subjects consistently found significant improvement in Asthma Control Test scores (SMD ~0.50) and FEV1 (SMD ~1.84), but the FEV1 confidence interval is extremely wide (0.07 to 3.60), indicating high heterogeneity and imprecision, and PEF showed no benefit. The only moderate-quality RCT (PMID 28093815, MRC-funded) showed a modest ~1.5-point ACT gain over placebo, while the others were low quality with undisclosed funding and heterogeneous, ill-defined preparations. Critically, every trial used black seed as an add-on to maintenance inhaled controller therapy, never as a replacement, and major asthma authorities (GINA, ATS) do not name or endorse it. Cleveland Clinic is the only consumer-health body addressing the pair and is only cautiously supportive, stressing it is not a substitute for controller medication. This is a textbook grade C: genuine signal exists but the body of evidence is small, inconsistent, and uncertain.

PMID: 28151459 2017 RCT (single-blind) n = 76
結論:FEF25-75% and FEV1 increased significantly in the 2 g/day group (p<0.05). FeNO and serum IgE decreased significantly in both treatment groups by 12 weeks (p<0.05). ACT scores improved significantly at 6 and 12 weeks in both groups (p<0.001, p<0.01). Fewer exacerbations in the 1 g/day group (p<0.05).
