The totality of evidence is against resveratrol as an anti-aging or longevity intervention in humans. No human RCT has ever used lifespan or any hard aging endpoint as a primary outcome; every positive longevity finding originates from yeast, nematodes, fruit flies, or mice, and an across-species meta-analysis (PMID 22718956) showed the effect diminishing and inconsistent in higher organisms. The NIA-led prospective cohort of ~800 older Italian adults found dietary resveratrol was NOT associated with reduced cardiovascular disease, cancer, inflammation, or longevity, and the NIA explicitly does not endorse supplementation. A 2021 systematic review and meta-analysis found no significant human effect on cognition, mood, grey matter, or blood pressure, and a double-blind RCT (PMID 31891746) showed a HARM signal — 1000 mg/day elevated CVD risk biomarkers (sVCAM-1, tPAI-1) in overweight older adults — consistent with the earlier finding that ~1000 mg/day blunts exercise adaptations. Poor oral bioavailability, no regulatory health-claim authorization (FDA, EFSA), and a SIRT1 hypothesis that has not translated to humans complete the picture. This is negligible-to-negative evidence, not merely uncertain evidence, which warrants grade D rather than C or U.
