Human RCTs and systematic reviews consistently confirm that NR safely and reliably raises blood NAD+ (around 2.6-fold), but this is a biomarker, not a clinical aging outcome. The clinical aging endpoints that actually matter (muscle mass and function, gait speed, grip strength, cognition, biological age) have NOT shown convincing benefit: a 2025 RCT meta-analysis on skeletal muscle was null on every endpoint, a 2023 high-quality critical review of 25 human studies found few clinically relevant effects and explicitly flagged literature-wide exaggeration, and the only biological-age signal comes from an underpowered n=20 ChromaDex-supplied pilot. No clinical authority endorses NR for aging: Harvard says it appears to have little benefit, Cleveland Clinic and GSA are cautionary, and NIA states no treatment is proven to slow aging. Grade C is warranted because evidence on the aging use-case exists and is actively studied but is small, surrogate-only, and inconsistent at the clinical level; it does not reach D because there is no convincing counter-evidence of harm and the NAD+ pharmacodynamic effect is real.
