The grade rests on one methodologically sound meta-analysis pooling nine randomized trials (PMID 24252493, n=437), which found Bacopa significantly shortened Trail Making Test B by 17.9 ms and choice reaction time by 10.6 ms (both p<0.001), improving speed of attention. Two direction-consistent RCTs reinforce this: a 12-week trial in older adults (PMID 18611150, n=54, 300 mg/day) improved delayed word recall and Stroop performance, and a 6-week student trial (PMID 27803728, n=60) showed measurable cognitive gains.

It lands at Preliminary rather than higher because the measured effects are small and of modest real-world significance, benefits generally require 12-plus weeks of chronic dosing, and the standardized extracts differ substantially between trials (bacosides 20% versus 50%, CDRI 08), limiting comparability. Funding was also undisclosed for the 2014 meta-analysis and the 2016 trial, leaving a potential conflict-of-interest gap.

Regulators and clinicians remain notably more cautious than this early research. The FDA recognizes no qualified health claim and has sent warning letters over cognitive disease claims; the EFSA classifies Bacopa as an on-hold botanical, neither approved nor rejected. The Alzheimer's Association states flatly that no supplement has been proven to benefit cognitive function or brain health, while Mayo Clinic, Cleveland Clinic, and Harvard Health carry no endorsing entry.
