Verdict: Promising in cognitive decline, unproven in healthy adults

Alpha-GPC has preliminary, mid-tier evidence: it improved cognition in older patients with dementia or cognitive dysfunction, but whether it sharpens thinking in healthy adults remains unproven. Treat "brain-boosting" marketing claims with caution.

This earns a Preliminary (B) grade because the supporting data are real but narrow. The strongest study, a 2023 systematic review and meta-analysis (PMID 36683513, n=861), found alpha-GPC improved cognitive scores (MMSE mean difference +3.50) in adults aged 50+ with dementia or cerebrovascular cognitive dysfunction. That population is the key caveat: those results do not automatically transfer to healthy people seeking a memory boost.

Evidence in healthy adults is thin and weak. A 2024 trial (PMID 39683633) gave just 20 healthy men a single dose and saw improvement only on the Stroop test, with no effect on N-Back or Flanker tasks; it was small, acute, and industry-funded, raising selective-reporting concerns. A 2021 trial (PMID 34207484, n=39) increased self-reported motivation but did not directly measure cognition. No high-quality, long-term RCT shows chronic supplementation improves thinking in healthy people.

Authorities reinforce the caution. The US FDA accepted alpha-GPC only as a GRAS choline source ('FDA has no questions') -- a safety, not efficacy, judgment, and the agency endorses no cognitive claim. The NIH Office of Dietary Supplements is cautious, and Cleveland Clinic, Mayo Clinic, and Harvard Health offer no positive efficacy recommendation. A drug-interaction flag also applies, so consult a clinician before use.
