Pomegranate for Cardiovascular Disease

Verdict: Weak, surrogate-only evidence; not a heart treatment

Pomegranate modestly lowers blood pressure and some inflammatory markers in trials, but no study shows it prevents heart attacks, strokes, or cardiovascular death, and its best-designed trial failed. It is not a substitute for proven heart therapies.

🔬Why this grade7-layer evidence engine

The grade is held to weak (Tier C) because every cardiovascular finding rests on surrogate markers rather than hard outcomes. Two meta-analyses do agree that pomegranate lowers blood pressure: across 8 RCTs systolic pressure fell about 5 mmHg (PMID 27888156), and across 22 RCTs about 8 mmHg (PMID 38410857), with a 16-RCT analysis also reducing hs-CRP, IL-6, and TNF-alpha (PMID 32147056). But these analyses carry very high heterogeneity and undisclosed funding, so an industry conflict of interest cannot be ruled out.

Crucially, the evidence weakens where it matters most. A 12-RCT lipid meta-analysis found no significant effect on cholesterol or triglycerides (PMID 26857863), and the only adequately powered trial of carotid wall thickness (PMID 19766760, n=289, 18 months) was negative on its primary endpoint, with benefit appearing only in post-hoc subgroups. The famous 30% improvement (PMID 15158307) came from just 10 people, unblinded and non-randomized.

Regulators reinforce the caution. The FDA issued a warning letter treating POM Wonderful's atherosclerosis and blood-pressure claims as unapproved drug claims, and EFSA rejected all related cardiovascular health claims for failing to establish cause and effect. The NHS frames pomegranate only as a superfood marketing term, and even Mayo Clinic calls the cholesterol data merely small studies. Pomegranate juice also inhibits CYP3A4/CYP2C9, with case reports involving warfarin and statins.
