Verdict: Weak, indirect evidence for anxiety

Bacopa monnieri may modestly ease stress-related symptoms, but the evidence for treating anxiety is weak: no trial was actually designed to test it as an anxiety treatment, so it should not be relied on for an anxiety disorder.

This grade reflects that no clinical trial has tested Bacopa specifically for anxiety; in every available study anxiety or stress was only a secondary outcome. In an elderly cognition trial (PMID 18611150, n=48), combined state-plus-trait anxiety scores fell with Bacopa and rose with placebo, but the report gives no p-value or effect size, so the size of any benefit is unknown.

The two other trials measured stress rather than clinical anxiety. A 2025 healthy-adult trial (PMID 41091332, n=87) found a greater drop in self-reported stress reactivity (p=0.03), but it used no validated anxiety scale and was industry-funded with author conflicts of interest. A small acute crossover study (PMID 23788517, n=17) lowered salivary cortisol, a physiological surrogate, without robust mood or anxiety effects.

No regulator or clinic endorses Bacopa for anxiety. The FDA has issued only warning letters against unproven disease claims, the EFSA classifies its botanical claims as on-hold, and the NHS, WHO, NIH ODS, Mayo Clinic, and psychiatric bodies offer no rating for this use. A plausible calming mechanism plus small, indirect, partly industry-funded trials supports a weak rating, not a recommendation.
