The one available synthesis, a 2025 systematic review of caffeine in cosmetic anti-hair-loss products (PMID 39997270, 587 participants across 9 trials), included only androgenetic alopecia, telogen effluvium, and general shedding. No randomized trial, cohort, or even case series has tested topical caffeine in alopecia areata, so there is nothing to confirm or refute a real effect.

Biology argues for caution about extrapolating. Caffeine's proposed action on hair is phosphodiesterase inhibition and cAMP signaling at the follicle. Alopecia areata is a CD8+ T-cell autoimmune attack on the hair follicle, a completely different mechanism, so positive results in pattern hair loss cannot be carried over to this condition.

Regulators and major clinics reinforce the gap. The US FDA treats any over-the-counter external product marketed as a hair grower or for hair-loss prevention as not generally recognized as safe and effective, and effectively an unapproved new drug. Mayo Clinic and Harvard Health discuss alopecia areata without ever listing caffeine; their treatment ladders run to corticosteroids, JAK inhibitors, topical immunotherapy, and adjunct minoxidil. Anyone with alopecia areata should see a dermatologist rather than rely on caffeine products.
