Pterostilbene for Aging

Verdict: Unverified for aging; mechanism only, no human proof

There is no human evidence that pterostilbene slows aging or extends lifespan; the case rests entirely on cell, fly, and rat studies, and the one human trial that exists measured heart-and-cholesterol markers, not aging, while flagging a worrying rise in LDL cholesterol. We rate it Unverified, not "it works."

The grade is Unverified because no human study has ever measured an aging outcome (an epigenetic clock, senescence marker, frailty, or lifespan) for pterostilbene. Two reviews summarize plausible anti-aging mechanisms (SIRT1/NRF2 activation, reduced oxidative stress, telomere and senescence effects), but both explicitly say human clinical validation is lacking (Li 2018, PMID 29210129; Dutta 2023, PMID 37979699). The supporting data are preclinical only: longer lifespan in fruit flies (Beghelli 2022, PMID 35222791) and shifts in aging-related gene expression in rat blood (Tello-Palencia 2024, PMID 39562169) — biologically interesting, but not transferable to people.

The single human RCT (Riche 2014, PMID 25057276; n=80, 7 weeks) was a cardio-metabolic trial, not an aging study. It lowered blood pressure but, with pterostilbene alone, raised LDL cholesterol by about 17 mg/dL — a signal that runs against the 'heart-healthy anti-aging' marketing. There are no meta-analyses. Pterostilbene is also a resveratrol derivative with antiplatelet activity, so a drug-interaction caution applies, especially for older adults on blood thinners or statins.

Authorities do not endorse it for aging. The US FDA has no GRAS record on file (only self-affirmed industry GRAS), and the EU treats it as a novel food that is not authorized for sale without pre-market approval; Health Canada permits it only as a general antioxidant supplement ingredient. NIH ODS, Mayo, Cleveland Clinic, and Harvard Health do not address it, and a leading geriatrics journal notes only limited, unconfirmed functional benefits for this class of NAD+-related supplements.

Pterostilbene on Metabolic Parameters: A Randomized, Double-Blind, and Placebo-Controlled Trial
