Efficacy of incretin-based therapies in obesity-related obstructive sleep apnea: a systematic review and meta-analysis of randomized controlled trials
Finding: Pooling 5 RCTs (n=1024), incretin-based therapies (incl. tirzepatide) reduced AHI by a mean difference of -11.61 events/h vs placebo (95% CI -22.91 to -0.31, p=0.046), confirming a clinically meaningful but heterogeneous reduction in OSA severity.
View on PubMed Glucagon-like peptide-1 receptor agonists for the treatment of obstructive sleep apnea: a meta-analysis
Finding: Across 6 trials (n=1067), incretin therapies reduced AHI by -9.48 events/h vs placebo (95% CI -12.56 to -6.40), with tirzepatide showing the largest subgroup effect (-21.86 events/h) versus liraglutide (-5.10 events/h).
View on PubMed Tirzepatide for the Treatment of Obstructive Sleep Apnea and Obesity (SURMOUNT-OSA)
Finding: In two phase-3 double-blind placebo-controlled trials (n=469; Trial 1 no-PAP, Trial 2 on-PAP), tirzepatide cut AHI by an estimated 20.0 events/h beyond placebo in Trial 1 (95% CI -25.8 to -14.2) and 23.8 events/h in Trial 2 (95% CI -29.6 to -17.9), both P<0.001.
View on PubMed Tirzepatide on obstructive sleep apnea-related cardiometabolic risk: secondary outcomes of the SURMOUNT-OSA randomized trial
Finding: In this prespecified secondary analysis of SURMOUNT-OSA (n=469), tirzepatide produced greater improvement in cardiometabolic risk factors than placebo, with combined weight-plus-OSA change significantly mediating the systolic blood pressure reduction (no significant mediation for diastolic BP).
View on PubMed Tirzepatide for sleep-disordered breathing in SURMOUNT-OSA: Time course and association with body weight
Finding: This SURMOUNT-OSA secondary analysis (n=469) showed AHI improved progressively over 52 weeks and tracked the magnitude of body-weight loss, indicating the OSA benefit is largely weight-mediated rather than independent of weight change.

Those treated with tirzepatide had a nearly 59% drop in their apnea-hypopnea index, which measures the number of times a person's breathing slows or stops during an hour of sleep. Additionally, those taking tirzepatide lost close to 20% of their body weight. Several patients lost between 50 and 60 pounds, which was enough to completely cure their sleep apnea. The FDA approval of Zepbound for moderate to severe obstructive sleep apnea in adults with obesity is a positive development for patients and clinicians.
