Effectiveness of dietary supplements for skin photoaging in healthy adults: a systematic review and meta-analysis of randomized controlled trials

Finding: This 22-RCT meta-analysis found collagen, flavanols and polyphenols effective against photoaging while hyaluronic acid, lycopene and carotenoids were not, but vitamin E (tocopherol/tocotrienol) was NOT among the supplements with enough qualifying RCTs to be analysed, so it provides no direct positive evidence for vitamin E.

Effects of tocotrienol on aging skin: A systematic review

Finding: Of 18 included studies, 17 were preclinical (13 in-vitro/cell + 4 animal) and only 1 was a human trial (Ismail 2009, ~12-20 topical subjects reporting improved hydration and reduced wrinkle appearance); authors conclude tocotrienol's anti-aging potential requires further investigation.

Prospective Randomized Controlled Trial on the Effects of Almonds on Facial Wrinkles and Pigmentation

Finding: Photographic wrinkle severity fell 15% (wk16) and 16% (wk24) and pigment intensity 20% in the almond arm versus control, but skin hydration and TEWL showed NO difference and the active is whole almonds (polyphenols/fatty acids/vitamin E), not vitamin E itself.

Photoprotection of UV-irradiated human skin: an antioxidative combination of vitamins E and C, carotenoids, selenium and proanthocyanidins
PMID: 12239424 2002 RCT (double-blind)
Finding: The clinical endpoint (minimal erythemal dose/erythema sensitivity) showed NO significant difference versus placebo (only a non-significant trend); only the surrogate biomarker MMP-1 differed significantly (p<0.05), and vitamin E was one of five combined antioxidants, not isolated.

A Randomized, Investigator-Blinded, Split-Face, Controlled Trial Assessing Efficacy and Satisfaction of CE Ferulic Serum Following Nonablative Fractional Fraxel Laser Treatment for Photoaging Skin in Chinese Population
PMID: 40414817 2025 RCT (open-label) n = 50
Finding: The C+E+ferulic side had a smaller Day-7 erythema increase than saline (0.04 vs 0.18, p=0.011) with better hydration and satisfaction, but this measures post-laser recovery over 7 days rather than anti-aging treatment, and vitamin E is an adjunct to vitamin C and ferulic acid.
