Pomegranate for Hypertension

Verdict: Likely lowers blood pressure modestly, evidence disputed

Pomegranate juice probably produces a small reduction in blood pressure, but the evidence is inconsistent and regulators have not endorsed it, so it is at best a minor dietary adjunct, never a replacement for prescribed antihypertensive therapy.

This earns a Preliminary (B) grade because three meta-analyses point the same direction yet the underlying data are shaky. The largest synthesis (PMID 38410857, 22 RCTs) found systolic BP fell about 7.9 mmHg and diastolic about 3.2 mmHg, with larger drops when baseline systolic BP exceeded 130. Earlier pooled analyses agree on magnitude: roughly 5 mmHg systolic in PMID 27888156 (8 RCTs) and PMID 37461211 (14 trials, n=573), the latter strongest at doses of 300 mL/day or less.

The grade is held back, and flagged 'disputed,' by real weaknesses. The 2024 meta-analysis carried extreme heterogeneity (I2 around 90%), undermining the pooled estimate, and a qualitative review (PMID 29173650, 8 RCTs, 619 people) judged the evidence weak because only a minority of trials reached significance. Funding across studies is largely undisclosed, so industry conflict of interest cannot be ruled out.

Regulators and clinics diverge sharply. EFSA rejected all pomegranate health claims for lack of established cause and effect, and the US FDA's 2010 warning letter to POM Wonderful named the blood-pressure claim an unapproved drug claim; the UK NHS treats 'superfood' as marketing and caps juice at 150 mL/day. Cleveland Clinic and Harvard Health are cautiously supportive. Pomegranate juice also inhibits CYP3A4/CYP2C9 and may add to antihypertensive drugs, so anyone on BP medication should consult a clinician.
