Verdict: Reishi for fatigue: weak, disputed, cancer-only evidence

Reishi (Ganoderma lucidum) is not established for fatigue. The only direct human evidence is a single small, low-quality trial in breast cancer patients, and there is essentially no research on everyday tiredness.

This claim earns a weak, disputed grade because the direct clinical evidence is thin and confined to one narrow population. The only trial measuring fatigue itself is an open-label pilot in 48 breast cancer patients on endocrine therapy (PMID 22203880), which reported improvement on the FACT-F fatigue subscale and lower inflammatory markers over four weeks. But it was unblinded, reported no effect size, did not disclose funding, and is rated low quality, so it cannot carry a firm conclusion.

The supporting Cochrane systematic review of reishi in cancer (PMID 22696372, 5 RCTs, n=373) found better quality of life in four trials but did not pool fatigue as a dedicated endpoint and judged the overall evidence insufficient. So even the best available synthesis does not actually test fatigue head-on.

Authorities give no real backing. The US FDA only states it 'has no questions' about a single reishi-derived beta-glucan ingredient, which is not an efficacy endorsement; the EU EFSA has left botanical claims on hold without authorizing them; and the NIH/NCI notes there are no studies of G. lucidum with measured cancer outcomes. Mayo Clinic, Cleveland Clinic, and Harvard Health do not address it for fatigue. Critically, all human data concern cancer-related fatigue, with no studies on general or everyday tiredness, so any benefit cannot be extrapolated beyond that specific clinical setting.
