Garcinia cambogia (hydroxycitric acid) shows only small, inconsistent effects on blood lipids and does not lower LDL cholesterol, the marker that matters most for heart disease. The evidence is weak and disputed, and no major medical society endorses it for cholesterol management, while regulators flag liver-safety concerns.

The strongest data come from a single 2024 meta-analysis of 14 RCTs (PMID 38151892, n=623), which found modest statistically significant reductions in total cholesterol (-6.76 mg/dL) and triglycerides (-24.21 mg/dL) plus a tiny HDL rise (+2.95 mg/dL). Critically, LDL cholesterol — the primary cardiovascular target — did not change (p=0.880). Effects appeared only after more than 8 weeks and are small in clinical terms.

Underlying trials are weak: they are few, small (PMID 24133059, n=43; PMID 24944404, n=39), short, and measured lipids only as secondary endpoints to weight or visceral fat, raising selective-reporting risk. In PMID 24133059 only triglycerides fell; PMID 24944404 found no significant lipid difference at all. This thin, conflicting base earns a weak (Tier C) grade rather than anything higher.

No specialty body — AHA, ACC, NLA, Mayo, Cleveland Clinic, or Harvard — recommends garcinia for cholesterol; clinics mention it only as a fat-burner ingredient. Regulators add safety concerns: the US FDA recalled all Hydroxycut products over liver injury, and the UK NHS calls such diet pills 'at best ineffective and sometimes can be harmful.' Given the unfavorable risk-benefit balance, it should not replace diet, lifestyle change, or statins.
