Effects of Probiotics, Prebiotics, and Synbiotics on Calcium Homeostasis and Bone Health With Aging: A Systematic Review
Finding: Pooled across 8 RCTs (n=564) supplementation significantly raised serum calcium (MD +0.52 mg/dL, 95% CI 0.38 to 0.66) but failed to support any benefit on BMD or bone-health parameters in middle-aged and older adults.

Finding: Pubertal adolescents on 8 g/day inulin-type fructans for 1 year had greater calcium absorption (+5.9%, p=0.04 at 1 yr) and greater gains in whole-body BMD (+0.015 g/cm2, p=0.01) and bone mineral content (+35 g, p=0.03) versus placebo.

Finding: Pooling 28 trials (n=3,615), gut-flora interventions improved BMD (SMD 0.47, 95% CI 0.28 to 0.66, p<0.001, I2=0%), but the analysis pools prebiotics together with probiotics/synbiotics so the estimate is not prebiotic-specific and evidence quality was moderate-to-very-low.
