Finding: Pooled across 17 RCTs (3631 outpatients) probiotics cut AAD from 17.7% to 8.0% (RR 0.49, 95% CI 0.36-0.66), but the only BB-12-containing subgroup (L. acidophilus La-5 + B. lactis Bb-12) was NOT significant (RR 0.79, 95% CI 0.47-1.33), unlike LGG (RR 0.29, 95% CI 0.15-0.57) and S. boulardii (RR 0.41, 95% CI 0.30-0.57).

Probiotic Yogurt for the Prevention of Antibiotic-associated Diarrhea in Adults: A Randomized Double-blind Placebo-controlled Trial (Velasco et al., J Clin Gastroenterol)

Finding: In 314 hospitalized adults (mean age 76y) AAD occurred in 23.0% of the BB-12-containing probiotic group vs 17.6% of placebo - numerically worse - with absolute risk reduction -5.35% (95% CI -15.4% to 4.7%, p=0.30), and no difference in diarrhea duration, bowel movements, or mortality; authors concluded the LA-5/BB-12/LC-01 mix has no effect on AAD.

Bifidobacterium animalis subsp. lactis BB-12 Protects against Antibiotic-Induced Functional and Compositional Changes in Human Fecal Microbiome (Merenstein et al., Nutrients)
PMID: 34444974 2021 RCT (open-label) n = 62
Finding: In 62 healthy adults (42 BB-12 yogurt, 20 control) on amoxicillin/clavulanate, BB-12 yogurt limited the antibiotic-induced drop in fecal acetate (recovered to -1.6% of baseline by day 30 vs sustained suppression in controls) and gave more stable Shannon diversity, but these are microbiome surrogate endpoints and the trial was not powered for, and did not demonstrate, a reduction in clinical AAD.
