The use of Garcinia extract (hydroxycitric acid) as a weight loss supplement: a systematic review and meta-analysis of RCTs
Finding: Onakpoya 2011 pooled 12 RCTs (n=706) and found Garcinia/HCA produced a small, statistically significant short-term weight reduction of -0.88 kg vs placebo (95% CI -1.75 to -0.00, p=0.05); the magnitude is clinically trivial and the authors conclude the effect is unlikely to be clinically relevant.

Finding: Semwal 2015 reviewed mechanistic and clinical evidence for Garcinia cambogia and concluded that although HCA inhibits ATP-citrate lyase and may modestly suppress appetite and de novo lipogenesis, clinical weight-loss trials show inconsistent and only marginal effects, with hepatotoxicity case reports raising safety concerns.

Finding: Heymsfield JAMA 1998 RCT (n=135) found no significant difference between Garcinia/HCA and placebo for weight loss (3.2 kg vs 4.1 kg, p=0.14) or fat-mass loss when combined with a high-fiber low-calorie diet — the largest and most rigorous single trial of its era was negative.
