Spironolactone versus placebo or in combination with steroids for hirsutism and/or acne
Finding: Across 9 small trials, 100 mg/day spironolactone for 6 months significantly improved hirsutism vs placebo (subjective hair-growth improvement OR 7.18, 95% CI 1.96-26.28; Ferriman-Gallwey WMD -7.20, 95% CI -10.98 to -3.42) and appeared more effective than finasteride, metformin and low-dose cyproterone acetate, but evidence for acne was insufficient.
View on PubMed Do Pleiotropic Effects of Spironolactone in Women with PCOS Make it More than an Anti-androgen? Evidence from a Systematic Review and Meta-analysis
Finding: Pooling 24 RCTs, spironolactone (100 mg/day) significantly reduced hirsutism in idiopathic hirsutism vs finasteride (MD -2.43, 95% CI -3.29 to -1.57) and vs cyproterone acetate (MD -1.18, 95% CI -2.10 to -0.26), but in PCOS women (50 mg/day vs metformin) showed NO significant Ferriman-Gallwey benefit (MD -0.61, 95% CI -1.76 to 0.54) and no effect on FSH, LH, menstrual cyclicity, BMI or HOMA-IR.
View on PubMed Short-Term, Low-Dose Spironolactone for Treatment of Hyperandrogenic Symptoms of Polycystic Ovary Syndrome—A Systematic Review
Finding: Of 5 open-label RCTs (3 meta-analysed), short-term low-dose spironolactone (monotherapy or combined with metformin) showed NO statistically significant difference vs metformin alone in reducing Ferriman-Gallwey scores, total testosterone or BMI; authors concluded current evidence shows no significant advantage and that larger, higher-quality trials are needed.
View on PubMed Metformin combined with spironolactone vs. metformin alone in polycystic ovary syndrome: a meta-analysis
Finding: Adding spironolactone to metformin significantly reduced BMI and total testosterone vs metformin alone, and (treatment >6 months) improved fasting glucose and insulin resistance with no increase in adverse events, but produced NO significant improvement in hirsutism score and no change in FSH or LH.
View on PubMed Comparative efficacy of statins, metformin, spironolactone and combined oral contraceptives in reducing testosterone levels in women with PCOS: a network meta-analysis
Finding: In a network meta-analysis of 9 RCTs (613 patients), atorvastatin was the most effective at lowering testosterone and was significantly superior to spironolactone monotherapy (MD -2.90, 95% CI -3.77 to -2.02) and to spironolactone plus metformin (MD -2.83, 95% CrI -3.80 to -1.87); spironolactone ranked among the less effective agents.
